Although this wasn't identical, I decided to end my search and just get a similar one. When I received this, I was pleasantly surprised to see that it was actually nicer and better quality than the one I was trying to match it to. This is a great frame for a good price.

There are 13 openings, 12 smaller openings for birth through 11 months, and a center larger opening for a 1 year photo. Frame has a high quality black velvet back with an easel for tabletop display & a hanger for wall mounting. Individually boxed. Each small opening is for a 1.5" x 2" photo, and the center opening is for a 3.5" x 5" photo.
